Engage with EmpowerMyCH through the mobile app on your smartphone or a web browser on your favorite device. Once you create an account, you can use either method to access EmpowerMyCH activities. The 'My Medical Passport' features are only available on the mobile app. However, if you like, you can build the passport from a web browser and, therefore, a computer or tablet for convenience.
You can build your medical passport, a hub for vital health information, in the third and fourth months after joining. Once you build your medical passport, it will be stored in the EmpowerMyCH mobile app, and you can access it wherever you go.
This is an entire community of other patients like you with whom you can engage. You will have opportunities to share your experiences and ask your questions to other patients while hearing from others and building a support network.
Quickly access the Adult Congenital Heart Association's (ACHA) provider directory in the mobile app. The ACHD Clinic Directory provides information on ACHD Board Certified cardiologists, certified by the American Board of Internal Medicine (ABIM), ensuring you have access to the specialized care you need for your condition.
Every few months, we will bring you insights from leading CHD experts and fellow patients, helping boost your confidence and knowledge in navigating your CHD. In the first month, you will explore essential standards of care and discover key considerations for assembling your ideal ACHD team.
